What rhinoplasty involves

Rhinoplasty reshapes the bone and cartilage of the nose to change its size, profile or tip. The most common requests are reduction of a hump on the bridge, refinement of the tip and narrowing of the nose; less commonly, the operation augments a small nose.

The nose is one of the most defining features of the face and has a disproportionate influence on how appearance is perceived. Where a patient finds it difficult to express what they would like changed, photographs taken at consultation are used to discuss what is and is not correctable.

Surgery is performed under general anaesthesia as a day case at Waterfront Private Hospital in Edinburgh and takes two to three hours. Open and closed rhinoplasty

Closed rhinoplasty

The closed approach places all incisions within the nostrils, leaving no visible scarring. It is suited to nasal hump reduction, nose narrowing, and refinement of the nasal tip, where the required work is more limited.

Open rhinoplasty

The open approach uses a small additional incision across the columella — the strip of skin between the nostrils — giving the surgeon full visibility of the underlying bone and cartilage. It is preferred when more intricate work is needed on the tip of the nose. The columellar scar typically settles to a fine line that is difficult to see at conversational distance.

Chin enhancement combined with rhinoplasty

Nose reshaping is often combined with chin enhancement. A weak chin can exaggerate the appearance of a prominent nose, particularly on the side profile, so a rhinoplasty alone in this setting may not achieve the balance the patient is seeking. This is assessed at consultation.

Correction of septal deviation (septorhinoplasty)

Rhinoplasty can be combined with a procedure to correct a deviated nasal septum where breathing is affected. This combined operation, called septorhinoplasty, is performed in one sitting. Rhinoplasty for purely functional reasons, without any aesthetic concern, is not performed at Quaba.

Non-surgical rhinoplasty

Some patients wishing to avoid surgery may be candidates for non-surgical rhinoplasty using dermal fillers. Fillers can soften small bumps, build up flat areas, increase tip projection, or straighten a nose. Results are not permanent, but the technique can show what surgery could achieve, or be repeated to avoid surgery altogether.

Benefits of rhinoplasty

Rhinoplasty can straighten a crooked nose, reduce a hump, refine a bulbous or drooping tip and bring the nose into proportion with the rest of the face. Where a deviated septum affects breathing, correcting it at the same operation can improve airflow as well as appearance.

  • Improved breathing where septal deviation is corrected at the same time

The major changes are visible as soon as the splint is removed at one week. The nose looks substantially settled at two to three weeks and continues to refine for many months; the tip takes its final position over three to six months, and the most subtle swelling can take up to 18 months to clear.

Results depend on skin thickness, the structure of the underlying bone and cartilage, and how the tissues heal. Your consultant will be realistic at consultation about what can be achieved for your nose.

Who is an ideal candidate for rhinoplasty?

A good candidate for rhinoplasty has a specific concern about the appearance of the nose, is in good physical and mental health, and is at least 18 years old with facial growth complete.

  • A clear concern – a hump, a wide or drooping tip, a crooked nose – that matches what examination finds
  • Good general health, with no conditions that impair healing
  • At least 18 years old; there is no upper age limit, although swelling settles more slowly in older age groups
  • Realistic expectations of what surgery can and cannot change

Patients whose distress is out of proportion to the examination findings may not be offered surgery; features of body dysmorphic disorder are screened for, and the section further down this page explains this in more detail. Where the concern is small, non-surgical rhinoplasty with dermal fillers may be a more proportionate first step.

Risks and considerations of rhinoplasty

Rhinoplasty is performed under general anaesthesia as a day case at Waterfront Private Hospital. The procedure takes around 2 to 3 hours. General anaesthesia carries risks including clots in the legs (deep vein thrombosis) and, very rarely, clots in the lungs (pulmonary embolus); several precautions are taken to minimise these. The risk of a life-threatening complication under general anaesthesia is approximately 1 in 100,000. The risks specific to rhinoplasty are below.

Bleeding

Bleeding can occur during or after the operation and rarely (less than 1 in 100) may require a return to theatre. Heavy bleeding may require a blood transfusion. Bleeding can also occur in the first few weeks after surgery if there is an infection affecting the lining of the nose. Rarely, blood can collect between the lining of the nose and the septum and become infected (septal haematoma or abscess: less than 2 in 100).

Infection

The surgical site can become infected, and this usually settles with antibiotics. Deep infection can also occur, and very rarely can lead to collapse of the bridge of the nose, requiring re-operation.

Swelling and bruising

Swelling and bruising are expected. Most swelling settles by six months. Residual swelling may be present up to 18 months after surgery, particularly at the tip of the nose.

Breathing difficulty

Breathing can occasionally be worsened, with a higher likelihood if pre-existing breathing problems or allergic rhinitis are present. In most patients with pre-existing breathing problems, the aim is to improve breathing after surgery rather than worsen it.

Scar

An open rhinoplasty leaves a permanent scar on the undersurface of the nose (the columella). Very rarely, this may be red and thickened. Scars from incisions inside the nose are hidden.

Numbness

Small areas of numbness in or around the nose are common and usually recover over time.

Reduced sense of smell

Uncommon (less than 1 in 100) and usually recovers with time.

Under- or over-correction

Removing too much or too little tissue, which may produce a shape different from the one planned at consultation.

Unsatisfactory cosmetic outcome

Realistic expectations of what can be achieved for your face and skin type are discussed at consultation. The healing process is difficult to predict, and the result can take up to 18 months to fully settle. A residual hump may return, nasal bones can thicken in patients with overactive bone healing, and scar tissue may prevent skin from shrinking to the new shape. Minor irregularities under the skin may be visible or palpable.

Perfection is rare in rhinoplasty. Revision surgery at no further charge is offered only to correct complications or outcomes that fall below what is expected of a typical result, not to refine minor imperfections.

Aftercare and recovery after rhinoplasty

You will be up and about the day after your surgery, though it takes a couple of weeks before you are entirely up to speed. Most patients describe rhinoplasty recovery as uncomfortable rather than painful; soft nasal packing is removed the morning after surgery, and oral pain relief is sufficient for most patients.

An external splint is worn over the nose for the first week to support the new shape as the underlying structures begin to heal. Bruising around the eyes and across the cheeks is common in the first week and settles over two to three weeks. Most patients feel comfortable in public, with light makeup, from around two weeks.

Light walking can be resumed within a few days. Strenuous exercise, contact sports, and any activity that risks a direct blow to the nose should be avoided for 6 weeks. Glasses cannot rest on the bridge of the nose for several weeks after the splint is removed; contact lenses or a temporary glasses bridge support are alternatives in the interim.

A nurse reviews you at one week and more often if needed during the early healing period. Your consultant will see you again around six months after the result has settled. In the meantime, your consultant is always available — if anything concerns you, they will respond directly and arrange to see you as soon as needed.

What is a rhinoplasty, and is it the same as a nose job?

Rhinoplasty is the medical name for the procedure most people refer to as a nose job. The two terms describe the same operation. It is a surgical procedure to reshape the nose, addressing concerns such as reducing the bridge, narrowing the nose, refining the tip, shortening the nose, or lifting a droopy tip.

Am I a good candidate for rhinoplasty?

If you have specific concerns about the appearance of your nose, are in good general health, and are willing to undergo surgery, it is worth booking a consultation to discuss what can be achieved. At consultation we examine your nose, discuss your concerns, and let you know what is and is not realistic for your anatomy.

What is the right age for rhinoplasty?

We prefer patients to be at least 18 years old, with facial skeletal growth complete. Emotional and social maturity matters just as much as physical readiness. There is no upper age limit; rhinoplasty in the 40s and 50s is common, provided general physical and mental health allow it. Recovery and residual swelling may take longer in older age groups.

Can rhinoplasty correct breathing problems?

Where breathing difficulty is caused by a structural issue inside the nose, most commonly a deviated septum, this can be addressed at the same operation as an aesthetic change. The combined procedure is called septorhinoplasty. Rhinoplasty for purely functional reasons, without any aesthetic concern, is not performed at Quaba.

Can I have rhinoplasty if I have already had a previous nose operation?

Yes. Mr Awf Quaba takes revision rhinoplasty cases where a previous operation has not produced the result the patient was looking for. Revision cases are more complex than primary rhinoplasty and are assessed individually at consultation. Cost is quoted separately.

Is rhinoplasty painful?

Most patients describe rhinoplasty as uncomfortable rather than painful. Our anaesthetists typically use total intravenous anaesthesia, which usually means little postoperative nausea, and oral pain relief is sufficient for most patients.

Do you use nasal packing?

Yes, but soft packing is used and it is removed the morning after surgery. Removal causes little discomfort at this stage.

Will there be a visible scar?

Yes. We see rhinoplasty patients from Glasgow, Aberdeen, and other parts of the UK. Rhinoplasty is a specialised procedure with relatively few high-volume practices in Scotland, and patients often travel for consultation and surgery.

Revision Rhinoplasty, I wish I’d done it sooner

I had a revision rhinoplasty with Mr Quaba & I’m now 8 months post op.
I can’t recommend Mr Quaba highly enough.
My initial rhinoplasty was carried out by another surgeon 16 years ago. I lost a lot of blood during the surgery & my whole face was swollen and bruised. There wasn’t any aftercare offered. My nose initially looked fine but over a number of years it collapsed to one side, looked very crooked/thin. I was very self conscious, I hated touching or looking at my nose.
I knew if I was going to have a revision surgery it had to be performed by someone very skilled. I researched many surgeon’s & Mr Quaba’s name kept coming up, so I booked my consultation.
I was blown away by how extensive Mr Quaba’s knowledge was, he discussed the structure of the nose with me at length, possibilities of grafts etc. Mr Quaba has a lovely, warm and caring manner that instantly put me at ease. He encouraged me to digest all of the information & not to rush into surgery.
I got the surgery a year later.
The experience was night and day compared to my initial surgery. I was very well looked after from the point of consultation through to my 6 month review.
I needed a lot of surgery on my nose as there were many issues caused by the initial surgery, and needed 2 grafts put in. Although such a complicated surgery was performed, my recovery was very straightforward. I had minimal bruising only under my eyes, minimal swelling & there were no issues with blood loss. This shows the level of skill and care that Mr Quaba applies in his surgeries. The aftercare I received from Mr Quaba and his team was fantastic. I’m very happy with the results. My nose looks natural and stronger. It just looks normal, which is all I’ve ever wanted. Most days I don’t think about it, I’m absolutely fine touching it. I don’t feel self conscious at all & it’s all thanks to Mr Quaba. I wish I’d done it sooner.
